Why rotation hygiene matters

Subcutaneous tissue responds to repeated mechanical trauma by depositing collagen and adipose tissue at the injection site, a condition known as lipohypertrophy. Injecting into these hardened areas changes the absorption rate of the compound, making dosing less predictable over time. Consistent rotation across a set of anatomical zones reduces the cumulative burden on any single area.

A common rotation framework for subcutaneous use divides the injectable surface into distinct quadrants or zones: abdomen (periumbilical area), outer thighs, deltoid region, and upper-outer glutes. Cycling through these zones and allowing at least 7 days before returning to the same spot is a widely cited practice for peptide and insulin-class compounds. Your clinical team may specify a different interval based on your injection frequency.
